The effects of temozolomide administered into brainstem by convection-enhanced delivery against rat brainstem GBM allograft models - Preliminary results

【Objective】The goal of this study is to evaluate the effects of temozolomide(TMZ) administration into brainstem by convection-enhanced delivery(CED) against rat brainstem GBM allograft models.【Materials and Methods】A total of 48 Fischer344 female rats were randomized into each group. Feasible studies of various doses of CED-TMZ into rats' brainstem had been performed and checked survival rates and neurotoxicity. Then, each 8 rats received an injection of 105 9L gliosarcoma cells. Five days after inoculations, total amount of 1mg of TMZ or saline was administered into brainstem using Alzet pumps(1ul/hr over 7 days). 8 other rats were treated by maximum dose of TMZ(50mg/kg/day) administered orally for 5 consecutive days. Survival was estimated and compared each other.【Results】All rats which low dose TMZ(1mg) administered into brainstem by CED over 7 days survived without neurological deficit, but in high dose(5-10mg) groups, serious neurotoxicity observed. Survival was significantly prolonged compared to control animals in low dose CED-TMZ group(median survival 23.5 vs 29.5 days, p<0.01). Maximum oral-TMZ administration group had a better prognosis compared to low dose CED-TMZ group(median survival 33.5 vs 29.5 days, p<0.01).【Conclusions】CED-TMZ is feasible and effective against rat brainstem GBM allograft. But CED-TMZ is not superior to maximum oral-TMZ. Modification of TMZ for injection and combination with other treatments are warranted for applying CED-TMZ into brainstem.
