A Case of Pilomyxoid Brainstem Astrocytoma

Objective
Pilomyxoid astrocytoma (PMA) is a recently described, variant of low-grade astrocytoma with unique pathological features and aggressive behavior. Most PMAs occurred in a significantly younger population and involved the hypothalamic/chismatic region. We report a case of PMA of the brainstem region in a 8-year-old girl.
Method
She presented with diplopia, facial asymmetry, and progressive right-side motor weakness. Brain MRI showed a rim-enhanced cystic mass, hypointense on T1WI and hyperintense on T2WI, measuring 3.0X2.0X3.6 cm with the epicenter in the midbrain. Stereotactic biopsy was done followed by radiotherapy. Histology confirmed the typical features of PMA. After partial resection of the tumor, chemotherapy was performed, and the tumor was reduced in size and became limited to the cerebral peduncle.
Conclusion
We believed that increased recognition of this rare astrocytoma could affect the treatment outcome of pediatric tumor patients.
